Support Systems Homes Relapse Prevention Program
This program is intended for those who are currently clean and sober, but have a history of relapse and want additional support. This includes those experiencing anxiety or fantasizing about returning to unhealthy behaviors after a period of time substance-free; or for those who have recently relapsed.
Support Systems Homes – Ongoing Support
The Support Systems Homes relapse prevention program consists of weekly groups facilitated by one of our counselors. While in the group, you’ll learn about relapse triggers, hidden warning signs, and what strategies you can use to support a healthy recovery.
Relapse prevention groups take place in our rehab center near downtown San Jose, California. This facility provides a welcoming environment that is easily accessible by car, bus, or light rail.
Participants are expected to actively engage in a group setting and complete written assignments. Participants should be willing to engage in some form of support network; and actively working to confront their issues while participating in this program.
Evidence-Based Curriculums Help Prevent Relapses
Our Relapse Prevention curriculum is based on the Gorski-Cenaps relapse prevention model, which uses evidence-based methods and approaches. These approaches have been tested over time and are shown to be effective in preventing relapse.
